TheNorth Carolina Office of State Archaeology (OSA) has developed a number offorms for the recording of historic and prehistoric archaeological sites. The forms have provide a standardized methodfor recording site information in a format suitable for computerized access andmanagement. This guide outlines the basic procedures for recordingarchaeological sites with these forms by professional archaeologists operatingthrough cultural resource management or academic research programs. In the following sections instructions are givenfor how to request permanent site numbers and how to fill out the appropriateforms.

REQUESTING PERMANENT NUMBERS.

Permanent numbers must be requested and assigned prior tosubmitting any archaeological site forms to the OSA. All requests should be in the format specified in the followingparagraphs.

Step 1: Assign each site a project specific, temporary site number toassist in proper identification for tracking of permanent numberassignments. Individual archaeologistsmay elect any number of possible arrangements.Examples include: A siterecorded during a fictitious survey of the Ararat River Valley might bedesignated as ARS89-7 (Ararat River Survey, 1989, #7); or by an individualamateur who refers to sites he has identified as PB-3 (Peter Brown, #3).

The only exception to these possibilities is the use of earlier,institutionally assigned Smithsonian format numbers. However, futher use of this format for temporary numbers willonly breed confusion between permanent and temporary designations. Even in the event a Smithsonian formattemporary number has been assigned to a site, investigators are urged to assignproject specific numbers in addition to the earlier reference.

Step 2: Prepare a copy of the USGS 7.5' (1:24000) topographic quad mapshowing the location of each site being reported and its appropriate temporarydesignation. Maps submitted should bein their original scale without enlargement or reduction. Additionally, if only a section of a USGSquad map is being submitted please indicate the quad map name and UTM zone fromwhich the section has been taken.

Please note that because of their lack of detailed topographic features andsmall scale, NC Department of Transportation County Road maps are unacceptablefor the location of archaeological sites.

Step 3: Submit your request with a cover letter specifying theorganization and project through which the sites were recorded.

Permanent number requests made according to thesespecifications can usually be processed the same day. All walk-in requests should follow the same specifications;however, because of scheduling conflicts assignments may not be available untilthe following working day. Requests areprocessed on a first come, first served basis.

REQUESTING PERMANENT ACCESSION NUMBERS.

The Office of State Archaeology will upon request assignpermanent accession numbers to site collections. Requests for accession numbers should be made at the same time aspermanent site numbers. Simply supply alist or indicate on the permanent number request which sites require accessionnumbers.

WHICH FORM TO USE.

<![if !supportEmptyParas]><![endif]>

Currently there are two forms used by the Office of StateArchaeology for the recording of site information. The forms and their intended applications are listed below:

Form III. This is the short archaeological site form III designed forspecialized information recording. Theprimary use of this form is:

a) To record all revisits and updatesto previously recorded sites that do not exceed the level of initial reconnaissanceinvestigations and to record isolated artifact finds.

Form VI. This is a longer, more general purpose form designed to recorddetailed site information for primary management and planning purposes. Form VI should be used under the followingconditions:

a) To record all previously unrecordedhistoric and prehistoric archaeological sites.

b) To record all site revisits orupdates that constitute significantincreases in the quality and/or quantity of site information. Instances that fall into this categoryinclude but are no limited to subsurface testingand excavation at the site.

c) To record all historic cemeteriesthat have been abandoned (withoutinterment) for a minimum of 15 years.

FILLING OUT THE FORMS

Whenfilling out either Form III or VI, it imperative that the information enteredis both legible and accurate. This isparticularly important since the forms are used for archival and researchpurposes. To ensure that each form isusable certain requirements are necessary as follows:

All information on forms filled inby hand, must be PRINTED inthe spaces provided on the form. Also,only BLACK ball-point ink isacceptable as other types of ink as well as pencil are illegible when the formsare microfilmed. Cursive handwriting isnot acceptable.

To ensure a standard of accuracy and minimal completenessfor each form submitted, certain fields must be filled with accurateinformation. Table 1 below identifiesthe various field numbers from Form V that are always required, the fields thatare required when applicable, and those that are optional. Forms that do not contain adequateinformation in the required fields will be returned for completion.

SITE FORM GUIDE -- FORM 6.

The following is a field by field guide to the type ofinformation required to accurately fill out Archaeological Site Form VI.

1.STATE SITE NUMBER: Thisfield is for the permanent site number assigned by the OSA, as the central sitedata repository (since January 1983).This should be filled in after permanent numbers have been obtained fromthe OSA.

2. SITE NAME(S): Record any name by which the site may beknown. In cases where there are two ormore site names, a "-" should be used to separate the first andsecond site names.

3.OTHER SITE #: This fieldprimarily applies to site numbers assigned prior to 1983 by institutions orindividuals other than the OSA. It isincluded on the form to allow cross-references to be made between these earlierdesignations and current permanent site numbers.

4.INSTITUTION ASSIGNING: Thisfield is for recording the name of the institution reporting the site. Please enter the appropriate code for thereporting institution given in Appendix A of this guide. If the reporting institution is not on thecurrent list, contact the OSA for an appropriate designation.

5.PROJECT SITE #: Individualarchaeologists may wish to identify sites by a specific project abbreviation orotherwise temporary designation.Assigning specific project numbers will allow retrieval of informationabout a particular project area with only a knowledge of the alphanumericprefix. Example: A site recorded during a survey of BladenLakes State Forest may be assigned the number BL77-142 (Bladen Lakes, 1977,site no. 142); information on all sites recorded during that survey would beretrieved by calling for BL77 data.

6.SITE COMPONENT: Circle thecategory that best describes the site as prehistoric or historic.

7.QUAD MAP: Using the list ofmaps provided in Appendix D, record the name, year and code of the USGSquadrangle map on which the site is located.If the name or code of your specific quad map is not listed in AppendixD, contact the OSA for an appropriate designation.

8.UTM DATA: Record the UTMzone and coordinates of the site being reported. The UTM Zone (either 16, 17 or 18) can be found in the lower leftor right hand corner of most quadrangle maps.The Northing and Easting coordinates should be read using the UTMtemplate provided with this guide by the OSA.When reporting UTM data please include the complete coordinates withoutabbreviations.

9.COUNTY: Enter county namefollowed by the appropriate two letter abbreviation code (See Appendix B) toindicate the county where the site is located.

10.DATE RECORDED: Use thisspace to record the MONTH, DAY, and YEAR on which the site was initiallyrecorded or visited.

11.SITE RECORDED AS RESULT OF COMPLIANCE PROJECT: By checking the appropriate space, indicateif the site was recorded as a result of a compliance project.

12.ER # / CH # /GRANT # (OBTAIN FROM OSA): If the site was recorded as a result of a compliance or grantproject, contact the OSA for an appropriate Environmental Review,Clearinghouse, or Grant number.

13.CODING DATE: Use this spaceto record the MONTH, DAY, and YEAR on which the site form was actuallycompleted.

14.REGISTER: OSA USE ONLY.

Archaeological sites reported duringSection 106 or similar compliance-related projects will be evaluated inaccordance with appropriate criteria for inclusion in the National Register ofHistoric Places. Investigators mustinclude NRHP significance recommendations in reports; final determinations will be entered in the state site inventoryby OSA personnel. For further referencesee: Guidelines for Preparation of ArchaeologicalReports, issued by the Office of State Archaeology and Division of Archivesand History, and National RegisterBulletin 15: "How to Apply the National Register Criteria forEvaluation" (National Park Service).

19.RESEARCH POTENTIAL: In thespace provided, and if necessary on an additional page, evaluate as succinctlyas possible the research potential of the site in terms of general and specificproblems of archaeological and anthropological method and theory. National Register of Historic Placescriteria of significance may or may not be of relevance.

20.POTENTIAL IMPACTS ON SITE -- ARTIFICIAL: Using the categories provided on the site form, circle the itemwhich best indicates the potential forartificial impacts on the site (i.e., from dam construction, increased areautilization, etc.). If none of thelisted categories adequately describes the artificial impacts, then selectOTHER and use the space provided for explanation.

21.POTENTIAL IMPACTS ON SITE -- ENVIRONMENTAL: Circle the item which best indicates thepotential for natural impacts on the site (e.g., erosion). If none of the listed categories adequatelydescribe the environmental impacts, then select OTHER and use the spaceprovided for explanation.

22.EXPLANATION OF IMPACTS:Briefly describe the causes of environmental and artificial impactsindicated in Items 20 and 21.

23.RECOMMENDATIONS: Circle theitem(s) which best describes recommendations for further action or research atthe site. If none of the listed itemspertain to the site, circle OTHER and use the space provided for a briefexplanation.

24.EXPLAIN RECOMMENDATIONS: Usethe space provided to record a more detailed but succinct explanation of therecommendations listed above.

25.DATE ON REGISTER: OSA USEONLY.

26-27.EXCAVATION DATE and INSTITUTION:Indicate the MONTH and YEAR when excavations were conducted. Additionally, record the institution and itsappropriate identification code. A codelisting is provided in Appendix A of this Guide. If the excavating institution is not listed, then contact the OSAfor an appropriate designation.

28.EXCAVATION RESULTS: Brieflydescribe the results of any excavation conducted at the site.

29.PERCENT DESTROYED: Circlethe estimated percentage of the total site that has been destroyed.

30.DATE DESTROYED: Record theMonth and Year in which the site wasdestroyed.

31.CAUSES OF DESTRUCTION:Circle the item or items which best describe Causes of Destruction atthe site. If none of the listedcategories adequately describe the circ*mstances, then select OTHER and use thespace provided for explanation.Additionally, if excavations have occurred at the site, they should belisted even if other types of site destruction have destroyed a greater portionof the site.

32.TOPOGRAPHIC SITUATION:Circle the landform category that best identifies the topographiclocation of the site. If none of theindicated categories fit the site's location, please indicate by circlingOTHER. Definitions for the categorieslisted on the form are provided in Appendix C of this guide and have been drawnprimarily from the American Geological Institute's 1972 edition of the"Glossary of Geology."

33.ELEVATION OF SITE: UsingUSGS 7.5' topographic maps, record the elevation of the site in feet or metersabove mean sea level.

34.SLOPE OF SITE: Record thepercentage of slope (not degree) atthe site location. This value isobtained by dividing the map contour interval by the distance between contourlines surrounding the site location and multiplying the result by 100. In the case where a site is situateddirectly on a contour line, a mean slope should be calculated by measuring theslope on each side of the site and averaging the two values.

Since many maps have contour intervals in feet, a slope calculation chart hasbeen provided below in Table 2. Toobtain a slope calculation for a site measure the distance between the contourlines surrounding the site. Using thismeasurement, select the column with the nearest matching distance and then findthe row that matches the map contour interval.The value indicated at this column/row position is the slope percent.

35.SLOPE FACE DIRECTION:Indicate the direction of the major downward slope at the site location.

36.SOIL COMPOSITION (SCS TYPOLOGY):Circle the soil composition category that best describes the soilspresent at the site location. If thesoil composition does not fit one of the categories provided on the form,please circle the OTHER category anddescribe the soil composition below in Item 26.

37.SOIL TYPE ABBREVIATION:Record the specific soil typeabbreviation, soil series name, and soil association name. This information may be obtained from U.S. SoilConservation Service soils maps. Ifdetailed maps are not available for the county where the site is located leavethis space blank.

38.MODERN VEGETATION: Circlethe category of vegetation currently covering the site. If none of the categories adequately describes the current site vegetation,please circle OTHER and describe the type of vegetation in the space provided.

39-40.DISTANCE TO NEAREST PERMANENT WATER FROM SITE: Approximate distance (in meters forprehistoric; in yards for historic) from site to nearest permanent water sourceidentified above.

41.TYPE OF NEAREST PERMANENT WATER:Circle the type of permanent water that is nearest to the site andrecord the name of that water source when available. Use the OTHER category for situations not described by thecategories listed. In the case of farmponds, canals, and other man-made bodies of water, this field should be leftblank. However, if the underlyingoriginal stream or water source can be identified from the USGS map then theoriginal water type and distance should be coded.

42.STREAM RANK: Record thehierarchical stream rank of the nearest water source. The ordering system used is based on Strahler (1957) and uses7.5' (1:24000) scale maps for order determinations.

According to the system 1st order streams are the fingertip tributaries at thehead of a stream and may either be year-round or seasonally flowingstreams. A 2nd order stream is formedby the confluence of two (2) 1st order streams; a 3rd order stream being formedby the confluence of two (2) 2nd order streams, etc. This system requires that at least two streams of a give order bejoined to form a stream of the next highest order. A given order, however, may have more than two lower ordersegments. The main stem of a river willalways be the highest order of the basin.

In general stream ranks on 7.5' topographic maps should vary from 1 to 6. In the event that the nearest permanentwater is a natural lake or other standing body of water such as a Carolina Bayplease code a stream rank of zero (0).

49.AREA COVERED IN CONTROLLED COLLECTION: Record the approximate area (in square meters) covered in anycontrolled collections of surface materials from the site.

50.SUBSURFACE TESTS: Indicateif sub-surface tests were conducted to determine the presence or absence ofsubsurface cultural deposits.

51.TEST METHODS: Indicate thetype of subsurface testing used at the site.If none of the listed categories adequately describe the methodologyused, then select OTHER and use the space provided for explanation.

52.DESCRIBE SUBSURFACE TEST RESULTS:Briefly summarize the results of any subsurface tests conducted at thesite. Please indicate the number ofpositive tests and the presence of any intact features or cultural strata.

61-66.INDICATE COMBINATIONS OF CERAMICS:These six fields are used to describe up to three different categoriesof prehistoric ceramics recovered from the site. Use the descriptions given to describe each category based on thetype of temper and surface treatments represented. Use Items 61 and 62 to record the primary ceramic present at thesite by circling the appropriate descriptions.Use items 63-66 to record any additional ceramics by entering theappropriate codes where necessary.
